Medical Records

To request medical records:

 

1) Click on the "Complete Secure Online Records Request" button below.  This will take you to a secure site where you can fill out and e-sign the records request form.  You will enter the numeric code shown and your email address and the form will open.  Please be sure to fill out the form completely and include the email address where you wish to have the records sent.  (This can be your email, or an email address given to you by your new physicians office)

2) After the request is received and processed, the email you designated to receive the records will receive two emails. One will be a temporary password for our HIPAA compliant secure email site.  The second will include the link to download your records.  Please monitor your spam or quarantine folders as these emails occasionally will be routed to those folders. 

 

3) Click on the link in the email to download your files and log in to the secure server using the provided temporary password.  You will be asked to create a new password.  After creating a new password you will be able to download your records.  All of your records will be sent in a single .zip file.  This file can be extracted using Mac or Windows operating systems.
